Your veterinary professional invites you to access and use this web site as a value-added service to provide companion-pet owners with a suite of communications and education tools, including electronic medical records, health articles and videos, breed information, virtual anatomy presentations and related functionality.

ePetHealth.com makes every effort to enable compliance with the medical records management principles of the
Principles of Veterinary Medical Ethics of the American Veterinary Medical Association (AVMA).

This Privacy Policy describes how ePetHealth.com, a service of Patterson Veterinary Supply, Inc. , collects information about you and your companion-pet(s), what we do with this information, and how we allow you to control this information. By using ePetHealth.com, you agree to the terms set out in this Privacy Policy

II. Registration
Your veterinary professional must invite you to access and use ePetHealth.com. You will be asked to provide a valid email address and to create a password upon first accessing the site. You will then receive an email that guides you through the remainder of the registration process.

By default, your veterinary professional will upload the following information about you and your companion-pet(s):

• Your full name;
• Your contact information;
• Your email address;
• Your phone number;
• Your companion-pet's name and demographic information; and
• Your companion-pet's medical record on file with your veterinary professional.

III. Collecting of Your Information
We may collect information about your companion-pet(s) when you or your veterinary professional updates your pet's medical record, initiates a prescription refill or an appointment, adds or modifies dosage alerts, or when you complete an online survey. We may collect information about you when you make changes to your profile on ePetHealth.com.

A. Provision of Username and Password We assign you a username, and you create a password to access ePetHealth.com. We strongly encourage you to choose a password that only you know and that would be extremely difficult for others to guess (such as using combinations of letters, numbers and special characters). You may change your password at any time.

B. Profile Information Once you start using ePetHealth.com, you may provide additional information (such as, cell, work and lost-and-found phone numbers) in the profile section. Providing additional information about you or your companion-pet(s) beyond what is minimally required at registration is entirely up to you. Any information you provide in the profile section may be used by ePetHealth.com and/or your veterinary professional as described in this Privacy Policy. If you decide not to share certain information, you may not receive the full benefits of being an ePetHealth.com user.

C. Customer Service We may collect information through ePetHealth.com in order to, among other things, accurately categorize and respond to customer inquiries and investigate breaches of this Privacy Policy. This includes email messages you send to us for support.

D. Site Usage We may collect information related to the tools and built-in applications you use on ePetHealth.com in order to assess the effectiveness of the site as a value-added resource for our users. For example, we may collect information regarding the videos you watch and the articles you read on the site.

E. Cookies We use cookies to track site usage and trends, to improve the quality of ePetHealth.com, and to customize your experience on the site. By using ePetHealth.com or adding any additional details regarding your companion-pet(s) or your profile, you expressly and voluntarily accept the terms and conditions of this Privacy Policy, allowing us to process information about you. Supplying information to ePetHealth.com, including any information deemed "private" or "sensitive" by you, others, or applicable law, is voluntary on your part. You have the right to withdraw your consent to our collection and processing of your information at any time, in accordance with the terms of this Privacy Policy, by closing your account. Please note that your withdrawal of consent cannot and will not be retroactive.

B. Site Operation and Service Delivery ePetHealth.com uses your information to operate the site and deliver the services or facilitate the transactions you request. For instance, we may use your information to provide effective customer service, make the site easier to use, perform research and analysis aimed at improving our products, services and technologies, and to display content customized to your interests and preferences.

We may provide aggregated anonymous data about the usage of the site to third parties, such as our web hosting and design partners, for purposes that we deem, in our sole discretion, to be appropriate.

E. Compliance with Legal Process We may need to disclose your companion-pet(s) information, profile information and/or information about your activities as a ePetHealth.com user when required by subpoena or other legal process, or if we have a good faith belief that disclosure is necessary to:

• Investigate, prevent or take action regarding suspected or actual illegal activities or to assist government enforcement agencies;
• Enforce our this Privacy Policy, to investigate and defend ourselves against any third party claims or allegations, or to protect the security or integrity of our site; and/or
• Exercise or protect the rights, property or personal safety of Patterson Veterinary Supply, Inc., our users, employees, agents, or others.

V. Control of Your Information
Except as otherwise described in this Privacy Policy, information about you and your companion-pet(s) will not be shared outside of Patterson Veterinary Supply, Inc. and its subsidiaries and affiliates without your permission.

VI. Closing Your Account
You can close your ePetHealth.com account by contacting your veterinary professional and requesting that your account be closed. If you close your ePetHealth.com account, your companion-pet(s) medical record(s) and your personal information will be removed from ePetHealth.com, but this information will not be removed from your veterinary professional's in-office medical record.
If you close your account, ePetHealth.com has no obligation to retain your information, and may delete any or all of your account information without liability. We may retain certain data contributed by you if we believe it may be necessary to prevent fraud or future abuse, or for legitimate business purposes, such as analysis of aggregated data, or if required by law.

VII. Children
Children may access ePetHealth.com only with permission from their parent or legal guardian.

VIII. Changes to this Privacy Policy
We may update this Privacy Policy at any time, with or without advance notice. In the event there are significant changes in the way we handle your information, or in the Privacy Policy document itself, we will display a conspicuous notice on ePetHealth.com and/or send you a service email. Unless stated otherwise, our current Privacy Policy applies to all information that ePetHealth.com has about you and your companion-pet(s).
By you using our Service after we provide a notice of changes, you consent to the changed terms or practices.
